Every level of care, on one chart

Crisis, inpatient, residential, outpatient, opioid treatment and recovery housing run on one set of records. A person keeps one chart as they move between levels, and every program posts to one ledger.

A sample chart for one person across five levels of care: a crisis alert on the first night, an inpatient admission the next morning, a residential level-of-care assessment and bed transfer in week two, an outpatient group session in week six and a move into a recovery residence in week nine.

What carries from one level to the next

When someone steps up or down, the next program opens the same record instead of starting a new one.

  • ASAM level-of-care assessments with a level-of-care history on each chart
  • One bed board across recovery residences, residential treatment facilities and inpatient units
  • 42 CFR Part 2 consent capture with redisclosure tracking
  • Suicide-risk screening and safety planning that follow the Zero Suicide framework
  • A general ledger with a fund structure for nonprofit (ASC 958) reporting
